A Chronic Condition Management Plan is a personalised plan created by you and your GP to manage long-term health conditions. Our clinic nurses can help you tailor the plan before your review with the GP. The plan includes setting personal goals to help you improve your health.

A chronic medical condition is defined as a condition that has been, or is likely to be, present for at least 6 months or is terminal. Talk to your GP regarding your eligibility.

A Chronic Condition Management Plan also facilitates access to Medicare benefits for up to 5 allied health services visits per calendar year to help you manage your condition.
