• We’re proud to announce the commencement of the Enhanced Integrated Primary Care (eIPC) program — a multi-disciplinary team (MDT) model focused on improving outcomes for people living with Heart Failure and Cardiovascular Disease.
• Working hand-in-hand with general practices and allied health providers, eIPC offers coordinated care and support to help patients better manage their cardiovascular conditions and overall wellbeing.

Enhanced Integrated Primary Care model delivers coordinated primary care for people with cardiovascular disease living in the Murray Mallee region. A specialist cardio nurse and social worker based at the Mannum Medical Clinic will work closely and coordinate support for patients through a multidisciplinary team involving general practice, allied health providers and mental health supports.
